Why should I have this document signed?

Signing your completed Koda Care Plan is required to make your medical care choices legally valid in your state in the form of an advance directive. By signing this document with witnesses, you are protecting your wishes and ensuring that your loved ones and providers know exactly what medical care you want. If the documents are not signed properly (or notarized), medical professionals won't be able to use them to provide care you might want.
