After completing your Koda care plan, you can download a copy and bring it to your next appointment. If Koda was introduced to you through a provider, the Koda team will send a copy to the associated provider when the care plan is completed and if the care plan is notarized with Koda Health. We recommend you provide a copy to your primary care doctor, any specialists you see, and any hospital systems you may be admitted to.
